Predictors of carotid atherosclerosis in essential hypertension.
Increased carotid intima-media thickness (IMT) is common in hypertensive patients. The mechanism is unclear but may be related to adverse clinical and subclinical vascular events in hypertensive patients. The aim of this study was to investigate the predictive factors of carotid atherosclerosis in essential hypertension. This was a cross-sectional study. Carotid ultrasound was performed on a total of 612 essential hypertensive patients who received antihypertensive drugs for at least 1 year. The intima-media thickness of carotid was measured bilaterally from the distal common carotid artery to the distal internal carotid artery. Carotid atherosclerosis was defined as an IMT ≥ 0.9 mm, and carotid plaques were defined as a focal thickening of the vessel wall, a protrusion or ulceration of the plaque into the vessel lumen, or thickening of the intima > 2.0 mm. The estimated glomerular filtration rate was calculated from the serum creatinine and age using the equation developed by the Modification of Diet in Renal Disease study. Logistic regression analysis was performed to determine the predictive factors for carotid atherosclerosis. A total of 290 (46.5%) patients were detected as having carotid atherosclerosis. After adjustment for age and estimated glomerular filtration rate, systolic blood pressure, and carotid plaques, odds ratios were 2.04 (95% confidence interval [CI] 1.48-2.82) for carotid plaques and 3.52 (95% CI 2.32-5.33) for carotid atherosclerosis.
